Greater Oakland Visiting Nurse Association (GOVNA) is the Skilled Care division of the company. Our referral sources will contact us, to provide Skilled Care to you or your loved one.
The referral can come from your Physician, a Case Manager or a Discharge Planner. They will contact our office, give us all the pertinent information needed for your care and request a date for start of service. GOVNA will contact the appropriate insurance company for verification, approval and on the requested number of visits. If additional visits are required, GOVNA will submit the proper documentation for re-certification. We will take care of it all. Our Registered Nurse will phone you or your family member to set up a day and time for the initial visit. At your home, we will complete a physical assessment and get your medical history. We will develop a plan of care to fit all of your needs.We will also provide reports and updates to your physician(s) as needed. Registered Nurses will also work as a team with the home health aide, Physical Therapist, Occupational Therapist, Social Worker, Case Manager, Discharge Manager and your Physician(s). |

Physical Therapist The therapist works with you to restore function, mobility, balance, work on pain control, range of motion and set up an exercise plan for you. Their goal is to improve your lifestyle and functional outcome as much as possible. Ordered by your physician as part of your discharge care or our skilled nurse can request an order for physical therapy after your initial home assessment. Once again, we will take care of all the required documentation, insurance approval and verification of visits requested. Occupational Therapy Assistance with the activities of daily living and their goal is to keep the patients as independent as possible. They care for a variety of patients suffering from many different conditions. All of your daily care needs can be addressed, such as bathing, dressing, brushing your teeth, eating, cooking, light housekeeping, computer use and other daily tasks. Social Worker The patients' advocate, they are there to help you overcome difficulties and improve your life. They have access to and work with various agencies in the community to seek solutions and remedies to assist you. They can offer direction, instruction and protection in certain circumstances for you or your family members |
